  • Clinical

    • primary thyroid lymphoma makes 5% of thyroid cancer
    • typically elderly women with Hashimotos
    • rapidly enlarging nodule

  • Cytology

    • monomorphic lymphoid cell population
    • MZL: small lymphoid cells (centrocytes, plasma cells, monocytoid B cells) interspersed with large lymphoid cells
    • DLBCL: large lymphoid cells (centroblasts, immunoblasts, Burkitt-like cells)
    • mixed

  • Ddx

    • Hashimoto thyroiditis: looks similar, can also contain clonal B cells, but clinically no mass
