Histology- purple crystals with strips (fish scales)
- ulcer, ischemic injury
Ddx- cholestyramine crystals (red, smooth, benign finding)
Signout- Fibrinopurulent exudate containing basophilic crystals
- Comment: Given the clinical history of recent sodium polystyrene sulfonate use, Kayexalate-associated ischemic hyperosmotic injury would be included in the differential diagnosis.
