  • Criteria

    • eosinophilia 1.5 E9
    • rule out other MPN (CML, PV, ET, PMF) or MDS/MPN
    • rule out PDGFRA, PDGFRB, FGFR1
    • rule out AML: blast < 20%, no inv16
    • clonal abnormality or blast > 2% (PB) or > 5% (BM)

  • Morphology

    • blood: eosinophilia (+/- reactive changes), neutrophilia, can have monocytosis and basophilia
    • marrow: hypercellular, eosinophilia with increased myeloblasts, charcot leiden crystals

  • Ddx

    • idiopathic hypereosinophilic syndrome (no clonality, no other cause, 6 mo duration)
    • reactive eosinophilia
    • other MPNs
