  • Histology

    • architecture: solid, sometimes glandular
    • thick walled blood vessels
    • cytology: prominent cell membrane (plant like), transparent or granular cytoplasm, perinuclear halo
    • key feature: the presence of nuclear membrane kink/irregularity
    • can have sarcomatoid dedifferentiation
    • eosinophilic variant chromophobe RCC
    • no need to grade, but note any sarcomatoid features
  • Stains

    • positive: Ecad (98%), Napsin (90%), Galectin (90%), CD117 (87%), CK7 (86%, diffuse)
    • negative: vimentin (2%), RCC (6%), CD15 (12%)
  • Molecular

    • extensive chromosomal loss
    • hypodiploid
