Clinical- bacterial
- males in pediatric, females in adults
- etiology: ascending (UTI) or hematogenous (endocarditis)
- microbiology
- ascending: E. coli (most common), proteus, klebsiella, enterobacter
- hematogenous: gram positives, fungi
- pyonephrosis = near-total destruction of obstructed kidney
- perinephric abscess = abscess in perinephric fat
- emphysematous pyelonephritis = gas bubbles in parenchyma
Gross- yellow-white foci or abscess
- ascending: pyramid > cortex
- hematogenous: cortex > pyramid
