  • Clinical

    • bacterial
    • males in pediatric, females in adults
    • etiology: ascending (UTI) or hematogenous (endocarditis)
    • microbiology
      • ascending: E. coli (most common), proteus, klebsiella, enterobacter
      • hematogenous: gram positives, fungi
    • pyonephrosis = near-total destruction of obstructed kidney
    • perinephric abscess = abscess in perinephric fat
    • emphysematous pyelonephritis = gas bubbles in parenchyma
  • Gross

    • yellow-white foci or abscess
    • ascending: pyramid > cortex
    • hematogenous: cortex > pyramid
  • Histology

    • PMNs
    • bacteria colonies
