MDS MPN unclassifiable

  • Criteria

    • Has both MDS and MPN features, but can't assign to a specific MDS/MPN category
    • No hx of MDS, MPN or therapy
    • Rule out AML, CML, PDGFRA/B or FGFR1

  • RARS with marked thrombocytosis

    • RARS: ring sideroblasts >= 15% of erythroids
    • thrombocytosis (plt >= 450) with abnormal megas
    • most have JAK2 V617F (60%) or MPL W515K/L mutation
    • exclude cases with
      • isolated del 5q
      • inv(3)q21q26
      • BCR-ABL1
      • prior MPN (progression or therapy changes)
