Names- lymphoplasmacytic lymphoma
Clinical- median age 60
- site: mostly marrow, sometimes node and spleen
- association: Waldenstroms (LPL, marrow involvement, IgM), HCV
Morphology- predominantly small lymphs mixed with plasmacytoid lymphs and plasma cells
- marrow: nodular, diffuse and/or interstitial infiltrate
- node: dilated sinuses, proliferation in intersinus regions
- monomorphic dark proliferation: lacks pale proliferation centers or marginal zone cells
- Dutcher bodies, cytoplasmic Ig
- increased mast cells
- can have paratrabecular aggregates
Stains- negative: CD5, CD10, CD23, CD103
- CD138 stains plasma cells
- strong surface and cytoplasmic Ig, usually IgM
